Please fill out the form below and begin with the child’s first name and one parents’ first name so It will be easy for Joseph to keep you in mind… Then please describe in detail the main issues your child or teen is facing: whether at home or in school, including
all his or her challenging behaviors
his or her emotional states
academic performance
the quality of his or her sleep
mealtime behavior
bath time behavior
relationships with both parents
Also describe sibling relationships and challenges if any…describe his or her friend situation…
Don’t forget his or her age and whether or not the parents have always been together or if they are separated and for how long, and how much time he or she spends with each parent.
Of course include any information you think is pertinent and any special situation such as an adoption, special medical needs or a religious or cultural situation that may affect your child.
The more information you give Joseph the better! This is an opportunity for you to express what is really keeping you up at night, to someone who is really going to listen and give you the results you need!
